Children diagnosed with a physical, developmental, or mental health related disability may be eligible for programs which are designed to support these children in their homes and in their communities. Individuals must have significant limitations in multiple daily living activities. Services are determined based on functional assessment of needs, identified goals, and outcomes. Children's Long Term Support, or CLTS, is an umbrella program for multiple programs.
