The Aging and Disability Resource Center (ADRC) provides information to the general public about services, resources, and programs for adults with disabilities, children with disabilities transitioning to adulthood, and the elderly in need of assistance.

  • Adult Protective Services/Elder Abuse referrals
  • Referrals for Elderly benefits counseling
  • Information on Home Health Services, Home-Delivered Meals, Caregiver
    Assistance, Transportation, Prescription Drug Coverage, Power of Attorney for

    Healthcare/Finance, Housing options
  • Medicare benefits and other health related benefits
  • Social Security benefits assistance
  • Disability benefits counseling
  • Support services and resources in your community

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA

Program Discrimination Complaint Form which can be obtained online at:

addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a

written description of the alleged discriminatory action in sufficient detail to inform the Assistant

 

 

Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The

completed AD-3027 form or letter must be submitted to:
